MVRS for all drivers, currently valued within 60 days

- Supplemental application and Acords accepted on 1-3 unit accounts
5 years of loss runs, if applicable, currently valued within 60 days

Blue Ridge Specialty application or comparable truck application
Equipment list reconciled to the most recent MCS-150
Driver schedule including years of experience
Five years of loss history (valued within 60 days of effective date)
Valid MVR’s (valued within the past 60 days)
0 – 24 power units, all driver MVRs required
25+, random sample of MVRs required for quoting

- Signed Rivington Application
- Signed UM/UIM coverage slection/rejection form
- Signed COE/Affidavit/Dilligent search form
- Any endorsement/exclusion that requires an insured’s signature
- Completed Quote
